For the safety of all participants and dogs attending training classes at Aqueduct Animal Hospital, I certify that my dog meets the following requirements: Current Rabies vaccination as required by New York State law, Current DA2PPV vaccination (Distemper, Adenovirus, Parvovirus, Parainfluenza), Current Bordetella vaccination, Negative fecal test within the past 12 months. My dog is free from contagious disease, including coughing, vomiting, diarrhea, parasites, or skin infections.
Proof of the required vaccinations and fecal testing must be submitted to Aqueduct Animal Hospital at least two (2) weeks prior to the start of the training session. If the dog is an existing patient of Aqueduct Animal Hospital, vaccination and fecal records may already be on file and additional documentation may not be required. Dogs whose vaccination or fecal records have not been received or verified by the required deadline may not be permitted to participate in the class. Owners agree to notify Aqueduct Animal Hospital if their dog develops signs of illness prior to a training class. Aqueduct Animal Hospital reserves the right to deny participation or remove any dog from class if health requirements are not met or if the dog appears ill.
Dogs participating in training classes must be at least 8 weeks of age and have received age-appropriate vaccinations recommended by a veterinarian. Aqueduct Animal Hospital reserves the right to determine whether a dog meets the minimum health and age requirements for participation.
Female dogs that are in heat (estrus) may not attend training classes due to the potential for disruption and stress among other dogs. Owners agree to notify Aqueduct Animal Hospital if their dog enters heat prior to a scheduled class. The clinic will make reasonable efforts to reschedule the dog for a future training session when available, but this may not always be possible.
For the safety of all dogs, owners, and staff: Dogs must remain on a NON-RETRACTABLE leash at all times unless instructed otherwise by the trainer. (Slip leads provided if needed). Owners must maintain control of their dog at all times. Dogs displaying aggressive behavior, excessive fear, or disruptive behavior may be removed from class at the discretion of Aqueduct Animal Hospital staff or the trainer. Examples of unacceptable behavior include but are not limited to: Biting or attempting to bite another dog or person Repeated lunging, snapping, or uncontrolled aggressive behavior Persistent disruption of the training environment Excessive Barking Aqueduct Animal Hospital reserves the right to dismiss any dog or owner from a class if their behavior poses a safety risk. No refunds will be issued in cases of dismissal due to unsafe behavior.
I acknowledge that: I am solely responsible for my dog's behavior and actions while attending training classes. I will follow all instructions provided by trainers and staff. I understand that dog training environments involve interaction with unfamiliar dogs and people, which carries inherent risks.
I understand and acknowledge that participation in dog training classes involves inherent risks, including but not limited to: Dog bites or scratches Injury caused by interactions between dogs or between dogs and people Damage to personal property By participating in training classes, I voluntarily assume all risks associated with participation for myself, my dog, and any individuals accompanying me.
In the event my dog becomes ill or injured during training classes and I cannot be reached immediately, I authorize the veterinarians at Aqueduct Animal Hospital to provide appropriate veterinary care at my expense.
Training class fees are non-refundable once the class session has begun. If an owner must cancel their participation before the start of the training session, Aqueduct Animal Hospital may offer a credit toward a future training class, at its discretion. Missed classes due to scheduling conflicts, illness, or other personal circumstances may not be eligible for refunds or make-up sessions. Make-up sessions are at the discretion of the trainer. Aqueduct Animal Hospital reserves the right to cancel or reschedule classes due to low enrollment, weather conditions, or unforeseen circumstances.
