Does the pet use a crate or kennel at home? How do they react? Have they ever caused harm to themselves trying to escape? (example: rubbing nose, broken tooth, bloodied paw or nail)

Are there any concerns with destructiveness when left alone? For example, have they ever chewed/ripped up/eaten toys, blankets, beds, etc.?

Is the pet sensitive or reactive to certain body parts being touched? Which are they?

Does the pet have any previous or current medical conditions or history?

If the pet is a rescue, is there any important known history that we should be aware of to make them feel safe and comfortable during their stay?

Which commands does your pet know? How reliable are those commands? Are there any special cues/commands you use that are not “common”?
